Human Factors Investigations (HFI)
Course Description
In the aviation industry human error is commonly associated with the nearly all aircraft accidents that occur. With such a high rate of human err as at least a partial cause in aircraft accidents, it has become essential for investigators to gain the necessary education and skills to both identify the human error issues that have played a role as well as to recognize when and how to utilize the required Human Factors experts for further analysis.
(L/D)max Aviation has designed the HFI course to provide investigators with pertinent information, tools and procedures that will aid in their investigations regarding human error in aircraft accidents. With a concise review of the role of human error in these accidents using SHELL, Reason and Helmreich models in combination with a taxonomy of unsafe acts, this course expands student's knowledge base of what needs to be evaluated in their investigations. There are a number of aspects regarding human error that need to be analyzed and researched. In this course, students will learn all about those various factors and how to optimally process an investigation to include the human factors involved.
In addition to the knowledge students will gain with regards to how to investigate an accident accounting for human error, it is critical to provide instruction and education of the requirements set forthwith regards to a general human error framework. Most accident databases that exist are not very helpful with regards to traditional human error analysis. A general human error framework is essential as it creates an opportunity for new investigative methods to be developed and current accident databases modernized.
The Human Factors Analysis and Classification System (HFACS) developed by Shappel and Wiegman, used extensively by the military, commercial and general aviation organizations, is outlined in depth in this course. The HFACS system is an invaluable aid in helping to look at and identify underlying human factors and to, therefore, help better aircraft accident investigations as a whole.
(L/D)max Aviation combines all of this knowledge and all of these methods into the HFI course provides students with the systematic framework and process necessary to accurately and comprehensively identify human error issues implicated in an aircraft accident. In addition, investigators who have taken this course will also have the knowledge needed to recognize the factors that may have led to those human errors.
Course Attendees
This course is has been developed to increase the knowledge of aircraft investigators on the issues surrounding human error in an aviation environment as well as the human decisions and limitations that sometimes result in accidents. Training investigators that are actively involved in the accident investigation on this topic provides the opportunity to continue to assess all of the variables involved in an accident and develop techniques to further prevent them in the future.

Administration
(L/D)max Aviation provides the HFI course as a 4.5 day endeavor incorporating training within the classroom. Students will reference lecture outlines and notes, pertinent reference materials and will receive a certificate of completion once the 36 hours designated for this course have been completed successfully.
